Your First Visit

On your first visit, plan to arrive 15 minutes before your appointment time in order to complete the necessary medical forms.

 

Please bring the following items with you:

  • A list of current prescriptions and over-the-counter medications you are taking, including dose and frequency.

  • Insurance Cards and PHOTO ID.

  • Any recent test results.

  • Referral forms, if required by your insurance company.

  • Please ask your Primary Care Provider to send or fax us a copy of your medical record before your appointment, so that our physicians are better able to assess your needs.
